Three Supplements, One Question

Protein, creatine, and omega-3 do not do the same job. A recent network meta-analysis sorts out which does what.

Most supplement questions get asked in the wrong shape. "Does it work?" assumes there is a single thing to work at. But strength, endurance, and recovery are three different outcomes, and a compound that helps one may do little for another. The more useful question is narrower: work at what, for whom, measured how.

A systematic review and network meta-analysis published in Nutrients in March 2026 set out to do something the supplement conversation rarely does: compare dietary protein, creatine, and omega-3 fatty acids head to head, across three separate outcomes — muscle strength, endurance performance, and recovery [4]. Most trials test one supplement against a placebo. A network meta-analysis pools direct and indirect comparisons so you can rank interventions that were never tested against each other in the same study, using a frequentist random-effects model to combine the evidence [4].

The inclusion criteria matter more than they might seem. The authors restricted the analysis to randomized controlled trials in people engaged in structured training for at least six months [4]. That is a deliberate filter. Results in trained bodies do not transfer cleanly to untrained ones, and vice versa — a novice often improves on almost any stimulus, which makes it hard to isolate what a supplement actually contributed. By requiring six months of structured training, the review is asking a specific question: among people already adapted to exercise, what do these three additions do?

The honest limit of what I can tell you from the available extract is that the review compared these three supplements across those three outcomes — not which came out ahead on each. That gap is worth sitting with rather than papering over. Because the more durable insight is structural: these are not interchangeable products competing for the same slot in your day. Protein supports tissue repair and the raw material for muscle. Creatine acts on short, high-intensity energy availability. Omega-3s sit closer to the inflammation and recovery side. Ranking them on a single axis of "best" misunderstands what each is for.

That framing connects to a broader point running through the recent literature. A 2026 review in Nutrients on nutrition in exercise makes the unglamorous case that nutrition affects all bodily processes by supporting energy production and tissue maintenance [2]. Supplements, in that light, are a thin layer on top of energy adequacy — useful only once the larger inputs are handled. And a December 2025 narrative review in the Strength and Conditioning Journal adds a caution that applies to every table in every supplement paper: many dietary and nutrient recommendations rest on research conducted only in men, even though physiological, neurological, and hormonal changes across a woman's life cycle meaningfully shift nutritional needs [1]. When you read a supplement ranking, it is worth asking who was in the trials.

There is also a safety dimension that the everyday framing of supplements tends to skip. A July 2026 episode of Sigma Nutrition Radio with pharmacist Giulia Guerrini made the point that supplement risk is not general but individual — the same product can be reasonable for one person and inappropriate for another depending on their medications, health conditions, dose, and formulation [3]. Some interactions change how a drug is absorbed, metabolised, or cleared; others amplify or oppose its clinical effect [3]. Creatine and omega-3s are among the most studied and generally well-tolerated compounds, but "well-tolerated" is a population statement, not a personal guarantee.

Put it together and the supplement question resolves into something calmer than the marketing suggests. Three of the best-evidenced compounds were serious enough to warrant a formal comparison across trained athletes [4]. That is a small, well-defined domain — not a mandate that everyone needs all three. The interesting work is matching the tool to the outcome you actually care about, on top of a diet that already covers energy and protein, and checking that nothing you take collides with anything you are prescribed.
